Circular(68)Issuing Of Dha Controlled Drug Prescription Pads

To:All Health Care Facilities -private Sector


Please be informed thatstarting from 3rd of April 2016, Dubai Health Authority will start issuingControlled Drug Prescription Pads for all private health facilities dealingwith the controlled drugs (CD).




Accordingto this, all heath facilities are requested to provide the followingrequirements to collect the prescription pads from the Health RegulationDepartment (HRD):

1.   A letter head from the health facility to HRD including:

a. List of the physicians’ names and their unique ID (mentioned on the DHA license)

 b. Authorized personnel to receive the prescription pads from the HRD (name & ID no. required). ID to be presented upon  receiving the pads

c. Number of prescription pads required

2    2. Handover the completed prescription pad(s) to the HRD

3payment of 50 Dirhams per pad
